Seniors Blue Book

The Seniors Blue Book is an excellent source of information for local senior citizens. Our office has free booklets of the Bergen County area Blue Book for anyone who wants them. Booklets are available in the waiting room or upon request from reception as long as supplies last. The link to the local website is below as well, for any patients who prefer online access.

Advance Directive Information

Advance Directive and Healthcare Proxy Information

Advance directives are legal documents that will communicate your end-of-life wishes in the event that you are unable to express your wishes. The advance directive is composed of a living will and a medical/healthcare power of attorney (may also be referred to as healthcare proxy).

National Hospice and Palliative Care Organization
